With advancing age, as a result of genetic factors and hormonal changes in the body coming together, the number of prostate cells begins to increase. This cellular proliferation process does not medically mean cancer; it is entirely benign enlargement. However, as the prostate increases in volume, due to its structural position it begins to compress the urinary channel running through it almost like a vise. Narrowing of the channel due to this external tissue pressure makes the task of the bladder, which stores and pumps urine, quite difficult.<\/p>\n<p>The bladder muscle is forced to generate much higher pressure than normal in order to push urine out through this narrowed channel. At first, the bladder tries to adapt to this situation and attempts to overcome the increased resistance by thickening its muscular structure. However, this difficult effort lasting for months and years can eventually cause the bladder muscle to become fatigued and lose its elasticity. The exhausted bladder can then begin to lose its ability to empty urine. The increasing need to get up at night to urinate both reduces sleep quality and leads to a constant feeling of fatigue during the day. During daytime hours, the frequency of going to the toilet may also increase. At the same time, it is quite common for urine not to start immediately when going to the toilet and to have to wait for a while before urination begins.<\/p>\n<p>As the degree of obstruction increases, a noticeable weakening in the strength and pressure of the urinary stream is observed. Urination that was once performed in a single strong stream and projected farther away may now begin to flow toward the feet, intermittently, drop by drop, or in a split stream. The process generally begins with a detailed examination aimed at understanding the patient\u2019s current condition. During the physical examination, the estimated size of the prostate, the structure of its tissue, and whether there is any suspicious hardness on its surface are evaluated. At the same time, a value called prostate-specific antigen is measured in the blood. This value, briefly known as PSA, provides important clues about the general health of the prostate gland and helps assess the possibility of potential risks other than benign enlargement.<\/p>\n<p>A urine flow test is performed to determine numerically the extent of obstruction in the urinary channel. The patient urinates into a special funnel-shaped device, and this device transfers the speed, duration, and pattern of urine flow graphically to a computer. Then, with an ultrasound examination, the estimated prostate volume in grams is calculated and the amount of urine remaining in the bladder after urination is measured. If the patient\u2019s complaints continue to increase despite medication use, or if certain side effects caused by the medications disturb the patient, the surgical option is considered.<\/p>\n<p>In addition, there are certain clinical conditions that can develop suddenly before medication is tried or while the patient is using medication, making surgery a necessity. The foremost of these is complete obstruction of the urinary tract. The patient suddenly becomes unable to urinate at all, experiences severe pain, and requires urgent catheter placement. If the obstruction continues even after the catheter is removed, surgery becomes necessary. In TUR-P operations, high-frequency electrical energy is used to cut tissue and stop bleeding. Depending on how this energy is transmitted to the tissue, the systems are basically divided into monopolar and bipolar methods. Since excessive fluid entering the bloodstream can alter the body\u2019s salt balance, the duration of monopolar surgery is generally preferred to be limited to a certain period of time.<\/p>\n<p>In bipolar or plasmakinetic systems, which are becoming increasingly widespread today, there is a much more advanced mechanism that protects the body. In this technology, electrical energy circulates only between two small points at the tip of the instrument and does not pass through the patient\u2019s body. Its greatest advantage is that physiological saline, simple salt water that is completely similar to our body fluids, is used as the irrigation fluid. When molecules in the saline meet the energy, an energy shield forms at the tip of the instrument. This shield cuts prostate tissue by vaporizing it rather than burning it with high heat, while at the same time gently sealing small blood vessels. Since physiological saline is used, even if the fluid enters the bloodstream, the body\u2019s sodium balance is not negatively affected. This provides the opportunity to comfortably clear larger prostates over a wider period of time and to complete the surgery with less bleeding.<\/p>\n<h2>What Is Considered During the Preparation Process Before TUR-P Surgery?<\/h2>\n<p>After the decision for surgery is made, a careful preparation phase begins so that the operation can proceed smoothly and recovery can be accelerated. One of the most important steps in this stage is making sure that there is no active infection in the urinary tract. If microorganisms are detected in urinalysis and culture tests, the infection is completely treated with appropriate antibiotics before the procedure. Since performing surgery in an infected environment carries the risk of microorganisms entering the bloodstream, obtaining sterile urine before the procedure is a basic rule.<\/p>\n<p>The patient\u2019s general health status, chronic diseases, and other medications being used are reviewed in detail. In older age groups, the management of blood-thinning medications frequently used due to high blood pressure, heart stents, or rhythm disorders is a critical issue. Since the prostate is a highly vascular organ, such medications are stopped a certain period before the operation with the approval of the relevant specialist doctors in order to manage the risk of bleeding. If necessary, shorter-acting injection treatments may be used during this period, protecting the patient\u2019s heart while also creating a safe environment for surgery.
