At this stage, non-contrast computed tomography is generally considered the most reliable imaging method for understanding the exact location, size, and structure of the stone. Thanks to tomography, the millimetric dimensions of the stone, its three-dimensional distribution within the kidney, the distance from the skin to the kidney, and the hardness of the stone can be revealed in detail. Knowing the hardness of the stone in advance helps plan how much laser energy may be used to break the stone during the operation. In addition, ultrasonography and X-ray imaging are used to confirm the degree of dilation in the urinary tract, largely clarifying the surgical strategy.<\/p>\n<p>It is extremely important that there is no active infection in the urinary tract before surgery in order to ensure a safe operative process. Therefore, urinalysis and urine culture are always evaluated. If bacterial growth is detected in the culture, the operation is generally postponed until the urinary tract has been cleared with appropriate antibiotic treatment. Then, with a very thin and flexible camera system whose tip can bend in every direction, the inner chambers of the kidney are entered one by one until the area containing the stone is reached. At the stage of stone fragmentation, high-technology laser systems are used. These systems attempt to dust the stone by turning it into millimetric grains of sand without causing it to jump from its position.<\/p>\n<p>While these dust-like fragments are passed comfortably and spontaneously through the urine, larger stone fragments that are too big to pass are captured with special umbrella-like wire baskets called &#8220;baskets&#8221; and removed. If the stone is located in the lower chambers of the kidney, which are very difficult to reach, it may be removed from its location with the help of a basket, transferred to the wider upper chambers of the kidney, and fragmented there. In this procedure, the patient is generally operated on under general anesthesia while lying face down or slightly on the side. Under the guidance of imaging devices in the operating room, a path is created by entering the kidney through the patient\u2019s back with a thin needle.<\/p>\n<p>With the help of guidewires passed through this needle, a thin tunnel extending from the skin to the kidney is created, and this tunnel is slightly widened with special instruments before an operative tube is inserted. The surgical team reaches the inside of the kidney directly using optical devices advanced through this tube. While somewhat wider sheaths were used in traditional procedures, today &#8220;mini&#8221; or &#8220;micro&#8221; methods using much thinner tubes aim to reduce the impact on kidney tissue, the risk of bleeding, and the likelihood of postoperative pain.<\/p>\n<p>Once the stones are reached, they are broken into pieces using various fragmentation instruments and are simultaneously removed by suction or with grasping instruments. These techniques are particularly prominent in patients with giant stones located in the renal pelvis and growing outward, or in patients with a congenital narrowing of the kidney drainage channel accompanying stone disease. The procedure is performed under general anesthesia using cameras and instruments advanced through several small openings made in the patient\u2019s abdomen or side.<\/p>\n<p>The tissues around the kidney are carefully separated and the renal pelvis is reached directly. The renal pelvis is opened with a small incision, and the large stone inside is removed whole in one piece without being fragmented. A notable aspect of this method is that it allows a large stone to be removed intact without spending time breaking it and cleaning out its small fragments. If the patient also has narrowing of the kidney drainage channel, the narrowing is repaired in the same session immediately after the stone is removed, helping the kidney return to healthy urine drainage.<\/p>\n<p>During this repair, the incision in the renal pelvis is closed with special surgical sutures that dissolve spontaneously inside the body. If the patient has skeletal system differences severe enough to prevent the advancement of closed instruments, or if closed methods have been tried and found insufficient, this method may be considered as a rescue option.<\/p>\n<p>Very rare situations in which open surgery may be considered include:<\/p>\n<ul>\n<li>Skeletal abnormalities<\/li>\n<li>Severe deformities<\/li>\n<li>Anatomical obstacles<\/li>\n<li>Failed closed interventions<\/li>\n<\/ul>\n<h2>How is it decided which kidney stone surgery method is suitable for which patient?<\/h2>\n<p>There is no single method that is suitable for everyone in stone treatment; an individualized strategy is developed according to the characteristics of the patient and the stone. When making this decision, the millimetric size of the stone, the kidney chamber in which it is located, the patient\u2019s additional diseases, weight, and general anatomical structure are evaluated as a whole. If the stone is relatively small, flexible laser surgery performed through the urinary tract is generally the first choice because it does not require any incision in the body and has less impact on the tissues. This method contributes to shortening the hospital stay and accelerating recovery.<\/p>\n<p>However, once the stone exceeds a certain size, the process of turning the stone into sand with a laser may take a very long time. Prolongation of the procedure may affect fluid circulation inside the kidney and change the likelihood of clearing the stone in a single session. Therefore, the PNL method performed through the back becomes a more frequently preferred option for large and staghorn stones. The location of the stone inside the kidney is also very important in the decision-making process. If the stone is located in the lower chamber of the kidney and the angle of this chamber is too narrow to allow the laser instrument to bend comfortably, the method involving entry through the back with a thin needle may be considered more appropriate.<\/p>\n<p>In patients who must use blood thinners or who have bleeding disorders, entering the kidney through the back may affect the bleeding risk, so laser surgery performed through natural pathways may be considered a safer alternative.
